If you have a current contact lens prescription and are ready to order, please fill out the form below. Once received, a member of our Prescott Valley team will personally review your request and follow up with you to confirm your prescription and finalize your order.
For those who need an updated prescription or are new to contacts, you can easily schedule an eye exam and contact lens fitting online.
